* Auto-detection of input format doesn't work for ovid and inspec.

* Upper-case words, i.e. acronyms, are not escaped by {..} in titles or
  anywhere else. (BibTeX output)

* Page ranges are not output with a -- between the numbers. (BibTeX output,
  field pages)
